faith, academic excellence, and community.
To complete your application, please be prepared to submit the following forms:
- Birth certificate (required)
- Baptismal certificate (if Catholic)
- Immunization records (required)
- Recent report card & standardized test scores required for 1st – 8th grade (State/iReady/Star)
- IEP or 504 Plan (if applicable)
- Teacher and Principal Recommendations
Please note that a complete student file is required before proceeding to the screening process. All new students will be on academic and behavioral probation during their first year as part of our commitment to their success.
